Rhesus factor (Rh + or Rh -) is a different way to classify the blood types….Erythrocytes have numerous proteins on its surface and rhesus factor is referring to a different antigen (protein) to that of the ABO system….

It won't matter accept in cases involving pregnancy. Should the rhesus factor of the foetus and the mother clash in a certain way, the mother's immune system will attack the foetus, treating it as a hostile invader. This is of course not a desirable outcome. However, your gynaecologist will advise you if this is a possibility, and there are medications to prevent this happening, so nowadays it's not so bad.

However, should you be planning a child, it is very advisable to know the exact blood type of your spouse so you will know whether the foetus will be at risk.

Basically, it's the same reason O- is regarded as the universal donor, and can be given to anyone. If O+ blood is given to a person with Rh - blood type, the person's body will typically treat the donor blood cells as invading microorganisms and have an immune response.
